[Genome] Conservated genes' GENEID

Hi,

I am now looking for human genes that are highly conservated. I got a
file from the UCSC Table browser using the following configurations:
group: comparative genomics
track: most conserved

and what i got is :
#bin	chrom	chromStart	chromEnd	name	score
1	chr1	25165819	25165859	lod=22	299
12	chr1	28311540	28311557	lod=14	244
12	chr1	29360117	29360136	lod=14	244
17	chr1	71303151	71303427	lod=462	674


Now what i need is to transform these data to the following format:

GENEID  score
8815          188
1981          800


the geneid is the id system used by the Entrez gene system. Or it can
be uniprot ac number.


I will appreciate if someone can offer me some tips on how to utilize
UCSC in this way.
